There were “several contributory factors” which led to the entirely “preventable implosion” of a submersible which cost five lives off Newfoundland two years ago, the coast guard investigators announced on Tuesday.
The Coast Guard has published its conclusions in a 300 -page report – the result of a two -year survey.
The diving scheduled for June 18, 2023 ended with a tragedy after the Titan lost communication with its Oceangate managers, triggering frantic research.
A few days later, it was determined that the submersible imploded and instantly killed the five people on board, officials said.
“This marine victim and the loss of five lives were avoidable,” said Jason Neubauer, president of the US Coast Guard Marine Board of Investigation responsible for surveying the Titan’s disaster.
“The two -year survey has identified multiple contributory factors that led to this tragedy, providing precious lessons learned to prevent a future event. It is necessary to monitor stronger and clear options for operators who explored new concepts outside the existing regulatory framework. ”
